Any proof of current work as the record of your

employment

(A copy of your business card, business permit,

Income Tax report, Copy of pay stub, Professional certificates/licenses)

3 high quality and up-to-date pictures

Pay membership Fee via Check, Credit, Debit,

Direct deposit, Venmo.

*Processing and Registration Fee: $200
